PUPIL RECORDS

 

The Board of Education of the Placentia-Yorba Linda Unified School District supports the need for pupil records as an essential part of the educational process. The board also recognizes that the handling and confidentiality of pupil records is important to students and their parents as well as to the school district.

Therefore, it is the policy of the school district that all policies and procedures pertinent to pupil records be in accordance with state and federal laws and regulations. Such policies and procedures shall:

  1. Guarantee access to authorized persons within five working days of the request to inspect and review records
  2. Assure security, including nondisclosure of personally identifiable information from pupil records without the prior written consent of the parent of the student or the eligible student
  3. Enumerate and describe pupil records collected and maintained
  4. Provide for the annual notification of the right to access by a parent or eligible student
  5. State that a nominal fee may be charged for copies of records
  6. Specify types and locations of records maintained and those responsible for those records
  7. Provide for an access log
  8. Provide for the correction or removal of information
  9. Provide for clarification of records
  10. Specify information that is to be designated as directory information

The Director of Special Pupil Services is designated as the custodian of pupil records for the district. Principals shall be responsible for the implementation of policies relating to the pupil records maintained at that school.

The custodian of pupil records is authorized to classify records in accordance with California Administrative Code Title V regulations and district regulations. However, permanent record classification beyond the requirements of Title V, Section 432, shall be submitted to the governing board for approval.
