REIMBURSEMENT OF EMPLOYEES FOR DAMAGE

OF PERSONAL APPAREL PROPERTY

 

A.      Purpose and Scope

1.      To outline the district commitment regarding reimbursement to employees for damage to personal apparel property suffered in the line of duty

2.      To specify the procedure an employee must follow to receive such reimbursement

B.     General

1.      Suggestions or questions concerning this procedure should be directed to the designated administrative unit responsible (see Section G).

2.      "Personal apparel property" is personal property normally worn or carried by the employee. Examples include eye glasses, hearing aids, watches, and articles of clothing.

3.      When personal apparel property is damaged or destroyed in the line of duty and without fault of the employee, the district shall make reimbursement as follows:

a.      The reimbursement shall be for the actual cost or repair or replacement above $20.00, and total reimbursement shall be established by the Board of Education.

b.      The right of the employee to reimbursement from the district shall be subordinate to the right of the employee to reimbursement from any other source or the right to reimbursement as damages from any individual.

C.     Note:  The district reserves the right to postpone or deny reimbursement until the employee has exhausted all other sources.

1.      Losses due to theft will not be reimbursed by the district.

2.      The Director of Business Services is responsible for the overall execution of this procedure as the district's claim representative.

D.     Forms Used and Additional References

None required. The claim is to be in letter form and is to follow the general format specified below (see Section D).

E.     Procedure

1.      The employee suffering the damage/destruction of personal apparel property is to prepare a letter of claim for repair or replacement directed to the district and to the attention of the Director of Business Services. The claim letter is to include the following:

a.      Detailed statement of loss describing damaged/destroyed item(s) and including cost estimates for repair/ replacement. Copies of any receipts and/or estimates from seller/repairman should be attached.

b.      Date, time, and place of incident resulting in loss

c.      Detailed statement of circumstances surrounding the loss

1)      What happened?

2)      Who was involved?

3)      Witnesses?

d.      Statement of employee's own insurance

1)      Is loss covered? To what degree? Policy number?

2)      Insurance company's name and address - include agent.

e.      Signature and title of immediate supervisor as approving the claim as stated

2.      On receiving the claim, the Director of Business Services is to investigate and to prepare recommendations for any required board action.

3.      The employee making the claim is to be notified in writing of the district's disposition or of postponement thereto and the reasons for it.

F.      Reports Required

1.      Letter of claim (see Section D.1)

2.      Recommendations for board action (see Section D.2)

3.      Letter of disposition (see Section D.3)

G.     Record Retention

The claim file is to be maintained for a period of three years after disposition, and then microfilmed prior to destruction.
